Today on Sunday’s Solutions we are going to expand on this past Friday’s, In-take.me, #patientchat** where I hosted Denise (@caregiving) of www.caregiving.com. The question of the show was, “Creating a Care Plan.”
Question:
Q1: What tools are in your caregiving plan that you can not do without?
Q1: Tools included in my care plan include, moms “1 page information sheet (includes: Her information, Emergency contacts “in order,” Insurance information, Medications “withe schedule”, Allergies. I include her medication schedule for the reason with my brother in-law and his medications you can change one thing about it and he can have side effects. If their on a schedule, make sure their kept on that schedule.
Q2: How often do you actually update your care plan?
A2: I update my care plan monthly and every time something happens to mom. The incident may change her medication, someone helping may not be able to anymore, medications change rapidly, etc. So you should constantly be reviewing your’s and your caree’s care plans.
